Here Toitū
Here Toitū is a free service that helps people with a health condition or disability to build confidence, improve their wellbeing, and find work, study, or volunteer opportunities.
We offer free support for up to 12 months, where our team will work alongside you to determine your goals and aspirations, and provide the support, guidance, and coordination to achieve these.
Here Toitū is delivered in partnership between ProCare Fresh Minds and the Ministry of Social Development.
There are two Here Toitū services; Standard and Responding Early.
Here Toitū Standard
This service can help you prepare for and find employment or study while living with a health condition or disability.
A Kaimanaaki/Health Navigator will work alongside you to identify your goals, create a personalised plan, and connect you with the support and opportunities that can help you succeed. This might look like arranging driving lessons, enrolling you in a course, or helping you create your CV and applying for jobs.
We will also introduce you to a Work and Income Advisor, who will help you manage your money.
Eligibility for the Here Toitū standard service:
Be between 18 and 64 years old
Currently receiving a main benefit
Have a Medical Certificate
Enrolled in a ProCare general practice in the Auckland region
Live in Auckland.
Here Toitū Responding Early
This service can help you if you have a job but might lose that job because of a health condition or disability.
A Kaimanaaki/Health Navigator and Work Retention Specialist will work alongside you to support your health and wellbeing, helping you stay in your current job or find employment that’s right for you.
You will also have access to a Work and Income MSD Advisor (Kaitohutohu) if you need financial assistance from Work and Income. Your team will make sure you’re getting the right health services and work with you every step of the way.
Eligibility for Here Toitū Responding Early:
Be between 16 and 64 years old
Be working full-time or part-time
Be living with a health condition or disability
Your health condition is impacting your ability to work and/or you are at risk of losing your job
Be enrolled with a general practice in the Auckland region
Not currently receiving a main benefit.
